The hardware is pretty much tested and proven. Things like that are largely operator error, it could be that they are letting the equipment get too dirty, or selecting the wrong ammunition for a suppressed application, etc. I've put quite a few rounds through that Five-seveN and SWR Spectre shown above, and not had any issues other than some blow-back material. There is a much higher level of cleaning maintenance needed when shooting suppressed. :beer:
